$50 New Patient Referral Bonus

At Orchard Maple Family Dental, the finest compliment that we can receive from you is an introduction to your family, friends and colleagues.  As a show of appreciation for referring new patients to us, we will give you $50 cash every time you refer a new patient to our practice.  In order for the patient to qualify for your referral bonus, they must pay in full for either a cleaning, operative, or cosmetic procedure.  The referred patient must write down the name of the person who referred them during their appointment.  Once the referred patient has completed treatment, the person referring them can come in to the office to receive their $50.

*Person referring a new patient cannot live at the same residence of the potential new patient.

**If the person referring a new patient refers a family outside of their own residence, the maximum amount they can receive for referring that family is $100, regardless of how many members are in that family.

$99 Whitening For Life

Upon completion of a comprehensive exam, necessary x-rays and your prescribed cleaning, you may enroll in our Whitening For Life! Program. A one-time fee of $99 gets you customized whitening trays and at-home whitening gel. Then at your regularly scheduled checkup and cleaning appointment (usually every 6 months), you will receive FREE touch-up whitening gel for your customized whitening trays.

Free In-Office Zoom Whitening

If you refer 4 new patients to our office, you will be eligible for free in office professional Zoom Whitening (A $299 value!).  In order for the patient to qualify for the referral, they must pay in full for either a cleaning, operative, or cosmetic procedure.  The referred patient also cannot use any other current specials in their treatment.

*Person referring a new patient cannot also receive $50 bonus if they are using referrals towards free in-office whitening.
